About the Role
Are you an experienced healthcare professional with a commitment to compassionate, patient-centred care? Do you have advanced clinical skills and a deep understanding of palliative care, or a desire to develop them? Then join us as an Advanced Clinical Practitioner, a brand-new role within the Hospice.
At Saint Catherines, our team is dedicated to providing dignified, holistic support for patients with complex health needs at the end of life. As an ACP, you will be a vital part of our interdisciplinary team, managing personalised care plans, assessing and treating symptoms and disease, and guiding patients and their families through their care journey. As a qualified ACP you will be expected to contribute to the out of hours medical 1st on call rota.
Key Responsibilities include:
- Conduct advanced clinical assessments, develop and implement comprehensive care plans, and provide direct patient care to manage symptoms effectively.
- Offer compassionate support, guidance, and information to patients and families regarding palliative care options and symptom management.
- Collaborate closely with our interdisciplinary team, including physicians, nurses, social workers, and allied health professionals, to ensure holistic, coordinated care.
- Contribute to clinical research, audit, and quality improvement initiatives to continuously enhance the care we provide.
About You
- Essential Qualifications: Registered healthcare professional (Nurse or AHP) with current registration and experience in a relevant clinical setting and a master’s degree in Advanced Clinical Practice or willingness to work towards.
- Experience: You have experience with complex care management, ideally within palliative, end-of-life care or frailty and demonstrate advanced assessment and clinical reasoning skills.
- Attributes: Compassionate, resilient, and adaptable, you’re committed to providing dignified, respectful, and empathetic care. You’re a strong team player with excellent communication skills and a genuine commitment to patients and their families.
- Knowledge and skills: You are competent within the capabilities for advanced clinical practice in England and the four pillars of advanced practice.
For candidates without the ACP qualification, we welcome applications from those who meet the essential experience requirements and are willing to undertake the masters level training to reach ACP level.
